Outpatient Support Services
NHYH has created an Outpatient Clinic that provides therapeutic and life skill activities. The Treatment Plan is utilized to identify specific programming that will be developed and followed to meet the clients goals. NHYH Staff team works with client’s CFT/Case Manager to determine appropriate services and frequency.
Community based counseling services is also an available component. All counseling services will be provided by a masters-level counselor. Counseling and Peer Support services can be extended to family members.
Additional services include mentoring, cultural awareness activities, after-school care, day life skills programming. The content of all programming includes Psycho/Social Rehabilitation, Client Wellness and Independent Living Skills objectives and goals.
